Signed authorization is required to transfer ownership of a Supra lockbox to another member. To maintain the security of the Supra system, this form is required by WSGVR. Telephone authorization cannot be accepted. 

If a Supra lockbox was originally purchased at another REALTOR® Association, it must be deleted from that Association's Supra database (or in some circumstances, deprogrammed) prior to being transferred to a WSGVR member.
